As a Presbyterian Medical Group patient, you have secure online access to your Presbyterian electronic health records through your MyChart account. You can also have access to your family's records by requesting proxy access and MyChart offers teens, between the ages of 14-17, access to directly engage with their care team. Plus, you can download the mobile app and manage your entire family's health care while on the go.

REQUESTING PROXY ACCESS
With proxy access, you can view certain medical information of your children or other adults.
For Your Child's Medical Records
If you are a parent or guardian of a minor child under the age of 14 and would like to request proxy access to their medical records, you will be required to have your own MyChart account.
From your MyChart account, you can request full view access by choosing Proxy Request from the menu. Below is the information you will need to have about your child and yourself in order to complete the request:
Full name
Date of birth
Address
Name of child's primary care provider
Last 4 digits of child's Social Security Number
Insurance member ID
The request will be reviewed and if determined to be valid, access will be granted and you will be notified via MyChart.
MyChart accounts for children between the ages of 14-17 can only be activated by that child. The teen can choose to grant limited view access to an adult. If a parent or legal guardian already has full view access to a child's account prior to their 14th birthday, their access will transition to limited view access on that birthday. Our policy follows New Mexico state law that patients age 14-17 have the legal right to consent to certain types of medical treatment without the consent of a parent or legal guardian. Presbyterian respects the rights of teen patients to keep their medical care private.
Minors age 14-17 with Complex Medical Conditions: The parent or legal guardian will need to message or call the provider to request full view access. Both the provider and the patient must agree to full view access. This is to support the teen’s right to confidentiality since full view access includes clinical information that may be sensitive.
On the child's 18th birthday, a parent or legal guardian's proxy access will be terminated. The adult patient (age 18+) can then grant limited or full view access using his/her MyChart account and selecting Share My Record from the menu.
For an Adult's Medical Records
If you are an adult and would like to provide limited or full view access to your medical records, you can do so from your MyChart account. Once logged in, choose Share My Record from the menu.

Getting Started
How do I register for MyChart?
Go to myPRES Registration
On the myPRES Registration page, enter your information and select Register.
When the MyChart box appears, select Activate MyChart.
Note: If you have not yet been seen at a Presbyterian facility but want to register for MyChart in anticipation of becoming a patient, you will need to call Customer Service at

You will see the home page of MyChart appear on your screen. You are now activated.

MyChart Features and Resources
How can I receive text message reminders from MyChart?
Opt-in to receive secure text messages for information about your care, including confirming or rescheduling appointments or reminders for important health needs. Just text START to 31082.
Text messages from Presbyterian Healthcare Services include updates related to your visits, MyChart account, one-time passcode, billing notifications, prescription reminders, and care management. Message and data rates may apply. Message frequency may vary. For questions, text HELP to 31082, or text STOP to opt out any time.
What is eCheck-In?
eCheck-In allows you to review and verify your patient information, electronically sign documents, and pay your co-pays before your appointment. Completing this process through MyChart saves you time at your visit.
How do I use eCheck-in?
A few days before your appointment, MyChart will send you an appointment reminder. Included in that reminder will be a link to complete eCheck-In. Just follow the prompts.
What is Hello Patient?
Hello Patient allows you to confirm your arrival for an appointment using your smartphone. This allows you to skip checking in at the front desk.
How do I set up Hello Patient?
Using the MyChart app, go to Account Settings and turn on Appointment Arrival. Before you arrive at the clinic, make sure you have location services turned on in your smartphone. When you arrive at the clinic, log into the MyChart app and once in the lobby you will receive a welcome notice. Just follow the prompts.
What is a New Appointment Offer?
For those patients who have been placed on a wait list, a New Appointment Offer is an automated notification of an opportunity to reschedule an upcoming appointment, if an earlier appointment becomes available.
How do I use New Appointment Offer?
If you are on a wait list for an earlier appointment, and one becomes available, you will be notified via email. You will be directed to sign in to your MyChart account to review the new appointment date and time. You will be given the option to accept the new appointment or keep your existing appointment. This offer is sent to several patients and is available on a first come, first serve basis. The first patient to accept the new time, will be rescheduled.
Technical Questions
What do I do if I’ve forgotten my password?
If you forget your myPRES password, use the “Forgot your password” link at the Sign In page. You will be asked to answer your challenge question and then your password will be sent to your email.
